$50 per month (or $575 if paid yearly)
Married Couples
$90 per month (or $1035 if paid yearly)
Families
(at least 1 adult member): maximum of $125 per month (or $1435 if paid yearly)
Member Scheduling Fee
There is a $20 scheduling fee for each member visit to help cover the cost of labs and supplies.
